OPTION B) Skip the call, and fill out the form below to get started with the health screening process.
Step 2:
InnerTrek will follow up with an email invitation to their services portal, where you will be invited to complete a Health and Information Form to determine eligibility and compatibility for this offering. The deadline to submit your interest is Sept 27.
Step 3:
Once your Health and Information Form is reviewed and approved, you can submit payment and we can begin the intake process.
